Example database

I would like an example of how a mini database works as I am trying to understand how rebodex works.

Could someone show me a code for simply 3 fields:

firstname
lastname
phone

I would just like it to simply write to a text file and display the results in the fields or a list...

I am sort of confused by teh panel thing so I am trying to break this into chunks.
